Verification Request

• Do you need to verify the status of your registration at CANNN with another nursing regulatory jurisdiction?
• Do you need proof of your exam results sent to another nursing regulatory jurisdiction?
• Do you need proof of previous or current registration with CANNN?

Please complete the following forms and submit to CANNN. Please note we only send verifications to a school or nursing or regulatory jurisdiction. 

The following documents are required.

1. Verification of Registration Request Form
2. Payment Authorization Form for all Transactions

Completed forms can be submitted to the Registration Coordinator info@rnantnu.ca

Please note that if you need a verification from another jurisdiction sent to us please visit the jurisdictional website to complete their request form and payment.
